Wheel spacers allow you to fine tune the off set of your wheels, either aftermarket or OEM. With aftermarket wheels they can also be used, as someone said, to push the wheels out if they hit the brake calipers. Going to a larger spacers may cause tire to fender clearance issues that may need addressed. This is done by trimming on a MINI because of the plastic fenders or by rolling (bending up) the metal fender lip on most other vehicles.
When getting wheel spacers be sure you get hub centric spacers, do not get the universal type as that will put all the stress on the wheel bolts or studs instead of the hub.

A wheels offset is about keeping the weight of the car and how it transfers it to the hub area in specs....a spacer effectivly changes this a bit, but still allows a different wheell to fit that may not met the orgional specs. With a mild, thin spacer, you likely will have minimal issues,just like cv joint life on a lowered car, but start to get agressive/extreme, and things happen....

I don't run them at present ( although my brake hats are a defacto spacer), but I will on my next set of wheels for the track, because of fitment issues, and it's a compromise, if I couldn't, I wouldn't. I will have them custom made hubcentric on one side and wheel centric on the other.
On my daily wheels, no spacers.
