Cooling fan on 90% and rough start
Did you replace / add the relay, check the sensor and also check to see if the coolant is low ( leaking thermostat housing ) ?

Have not replaced/added relay, is it a relay that is supposed to be there? Coolant is not low I checked it multiple times.
Also fan runs constantly even with A/C off and never being turned on during drive.

Most I have seen have it , I dont know why you wouldn't , worth a shot.
