Anyone had issues with Manic Stage 2?

Background on the car:
2011 John Cooper Works Hardtop with the N14. Has 106k miles. Manic tune applied around 100k miles. In the last 6k miles I have performed the following services: Timing chain kit replacement, vacuum pump replacement, 1 step colder plugs, new ignition coils, walnut blast on the intake valves, oil change, and air filter cleaning and oil.

The issue I am seeing is that around 3k RPM when rolling into the throttle the car will misfire and go into partial limp mode (only building ~1-2 psi of boost) if I turned the car off, take the key out, and then place the key back in the ignition and start the car and hit boost it comes out of it. It does not seem to matter the EGT or air intake temperature or ambient air temperature. I replace the timing chain to see if maybe it was getting out of time, no dice, replaced my plugs to OEM ones, no dice. Replaced the plugs with a step colder and again, no dice, so I ordered some Bosch coils and stock plugs and issue was resolved. It stayed this way for a week and a half (600 miles max) and then parked for a week while I traveled for work. Driving the hour home from the airport it started to miss out under normal driving, not bad enough it didn't come out of it, but enough to know. Ordered some Delphi coils and this did not fix the issue. Pulled the plugs and a couple were fouled so threw the colder ones back in and everything was fine. Now, ~1500 miles (colder plugs probably have 3k miles on them at this point) and it is now starting to misfire rolling into the throttle around 3k RPM again. Not bad enough it actually bogs the car yet, but enough to know it is there.

*side note* putting the car back on the stock map it does not have the issue, but also doesn't build nearly the boost it does on Stage 2 A, B, or C.

Supporting mods:
Miltek catless downpipe, larger front mount IC, Alta intake, Forged intake pipe, Forged hot side charge pipe, resonator delete. I do have a P3 gauge and my AFR reads around 14.2 at idle and dips to about 12.8-13.4 under load normally. Highway driving it may get into the low 15s. Nothing to me seems concerning there. The only time the CEL comes on is if it goes into partial limp mode.

Anyone seen this behavior before?

If it goes into limp mode, there should be a code stored in the ECU... What code is coming up?

I have a Manic ECU tune installed (not a clip on box) and have never had something like that happen. I may have the Stage 1 tune though and have an N18 JCW.

I need to pull the codes since the latest incident, however it was a Super Knock code and random misfire in all cylinders. There is also a decent amount of black smoke under heavy acceleration(see video below). Is it possibly running too rich, thus fouling my plugs, or is this normal on a decatted Mini?

Had a leaking injector so replaced the injectors this past weekend and some fresh plugs and issue persists still. Below are the most recent codes since the injector replacement:
2D52 DME: Super Knocking
2DCA DME: Fuel shutoff due to super knocking Cyl 4
2DCB DME Fuel shutoff due to super knocking Cyl 2
2DCD DME: Fuel shutoff due to super knocking Cyl 3
28AA DME: Bypass blow-off valve, plausibility (diverter valve probably has about 12k miles on it since replacement)
2DCC DME: Fuel shutoff due to super knocking Cyl 1
2FD7 DME: EWS anti-tampering protection

Any ideas on what else to look at next?

Are you using the correct gas for the tune?
Have you check psi on HPFP?

Yes, I have it tuned for 93 and I am using 93. Have used 93 across various gas stations and nothing. Even tried octane booster to be sure and same issue resulted.

Did some data logging today, trying to get the psi currently.
If that data wasn’t gathered I can get that.

Fuel pressure numbers varied from 5-12mpa (725psi to 1700psi)

change the fuel filter.
do not use die electric grease on the plugs which should be ngk iridium
check the turbocharger actuator to see if it's sticking!!!!!!!
Whenever I got super knocking from ALL CYLINDERS usually number 3 is to blame.
