Set the phone to First Name Last Name and then "delete the pairing on both the phone and car then "re Pair" the phone and car and see if that helps.
That should clear any contact list in the car which my not be updating since it only looks for new adds each time it connects.

It was in the correct order before I took the car in for service and they wiped my phone pairing.
edit: took it back to the dealer. The service tech couldn't figure it out so we paired his phone. It shows up with first name first. Deleted the pairing on my car and on my phone and tried again, still have the same issue. We are running the same version of iOS so now I'm really lost.

There should be two settings to change on the phone. both are in
Settings > Mail > Contacts > Calendar > Sort Order and Display Order.
Make sure both are the way you want it Last,First or First,Last

We are both running 5.1.1. I was running 5.0.1 when the issue first started and tried to see if upgrading would help.
Yes, it is in the correct order here. I've tried changing it back and forth in the phone settings but in the car it ALWAYS appears as last, first.
Ask MINI responded to my question by telling me to take it to the dealer, which wasn't helpful considering in my question I stated I had already done so.

My next suggestion will take some time to complete so do it when you have half a day or more to test with.
1. Delete pairings in both the phone and the car.
2. disconnect the battery for at least 30 minutes. This should reset all the cars computers to their normal settings. I.E. making sure the pairing data is deleted.
3. Hold both the Home Button and Sleep buttons on the iPhone until the apple appears on the screen. This is a hard reset of the phone that should also clear any residual pairing data.
4. Set up the pairing from phone to car again.
if this does not work then i am totally lost as to why it is not working for you.

I also tried pairing it using my other key, which has never had a phone paired using it. Didn't work either.
