Trailing arm bolt failure and brushing insets

On Saturday night my drivers side trailing arm bolt and arm pull out. I think that the bolt may have been loose, i had a light rattle that i could not diagnose. i was thankfully in town and going slow.

I installed the poly brushing inserts 2 years ago, the dealer said they did not know the cause of the failure and recommended the brushing as a possible cause.

has anyone had this issue? should i just have them take the inserts out?
