what kind of dash trim is this?
i really like it but don't know what it is... is it aftermarket part? anyone?

Piano Black. They began offering it in 2005+ I believe. It was a factory option.

It was an OEM option called 'Piano Dash' and came complete with those circle graphics and everything.

how many pieces is it? do they make it to fit a 5 piece dash?

They are 3 piece....they don't offer it in 5 because it came out after they had already changed to the 3 piece dash.
