Mike at Aesthetic Creations makes them for the warning label covers.
If those aren't the right size (too tired to go out and measure them right now), then he can easily make some custom decals for you.
He's a vendor on the site. Second vendor listed in the Vendor Directory (see that little link down there to your left ?).
