Grinding Gears (And other issues)

So when I am shifting from any gear into a higher one, wether its 2nd to 3rd or 4th to 5th, sometimes the gears grind. I always put the clutch fully to the floor, and I don't shift at a high RPM, or keep the gas on. Also, sometimes instead of smoothly going into 4th, it takes a little bit of force, not a lot, but just a bit more than all the other gears. Another thing is does is when I'm just driving around, it will randomly pop out of 5th, and sometimes it does it with medium accelleration in 3rd. Like the car has a mind of its own. Has anyone else had these problems? I had all the cables replaced, and I am wondering if anyone else has had these same problems with their car.

My 2nd gear grinds, could be bad syncros in yours.

Have you changed out the transmission fluid yet?

Yeah, I had a new clutch put in about 5 months ago. They changed the fluid then. I don't know what kind they put in though. Could that make a difference?
