Rev limited to 4k after start

This is a new one for me. Stock engine, R59S, 60k miles, intermittent cel. Cars starts as it should. My son drove it last night and called telling me there was a cel. He brought the car home (ten minutes away) with no issues other than the light. I plugged in the reader (Autel) and can’t find anything definitive. The cel has no come back on, but I noticed the car will not rev beyond 4k rpm. Odd thing is when I switch off the engine and crank it, if I immediately rev it, it goes to redline. Afterwards it won’t pass 4k again. I cleaned the maf and checked the air filter. All good. Still won’t go past 4k except immediate on startup. Plugs and coils have around 2k miles on them. Oil level is good (oil less than 1k miles). Engine not overheating.

I tried switching off all the traction control and shifting manually, and it will rev as normal through the gears up to 6k rpm in that particular configuration, but it feels like the power is limited - not drastic, but not normal.

I’ve been chasing this for about four hours now, but google has only suggested the usual culprits. I still need to check the O2 sensors. The reader indicates a lean fuel condition. Here is a quick image of the three issues showing on the reader…

Any thoughts at all are greatly appreciated! Trying to identify so I can avoid any unnecessary shop costs.

4K rpm is apparently the factory-programmed stationary soft limiter for your car. It's normal.

Your codes suggest a vacuum leak (PCV, dipstick, oil cap?) and/or boost leak (cracked turbo inlet?).

Ignore the downstream O2 sensor code until the above issue(s) is solved.
