What brand T mobile phone is it, and does the phone have all the current updates?? Might need to be updated.

Cant help without Make, Model and Firmware (software version) of the phone.

BTW if it is a LG phone try another brand they are notorious for being a problem with the MINI and many other OEM Bluetooth Systems

Looks like you might have had a software upgrade on the phone that has caused some of the problems.

First thing to try is to completely delete all pairings in the phone and the car. Then pull the battery from the phone to cause it to force reset. Then redo all the pairings for any bluetooth items you connect to.
