Most companies require a minimum order to cover the cost of setting up the design, but once setup individual shirts can be ordered as one offs.
My bike club has used both VGear and Canari.
With both you select one of their shirts and they apply your design.
VGear is cheaper, ~$45 per shirt, thicker, and not as nice. However they are the perfect weight for SoCal "winters", I.e. up to the high 60°s F.
Canari makes a better, summer weight shirt, but we found their customer service sucks and they approach $70 per shirt. The Canari full kit is nice though.
The VGear "club" fit is just a tad shorter than the Canari "club" shirts, but otherwise we found both fit true to size.
No one in my group is skinny, so I can't speak to the "euro" fit sizing.
My old mountain bike group simply purchased some Fox cycling jerseys from SuperGo (remember them?) and then went to a local silk screener. That's still my favorite shirt.
